Hi,
I would use a DH05C for the 218 and an FH05 for the "carrozza pilota". You can configure both to the same address, then you can use the front and back light in either direction.

I used a cheap "FD-R Basic" from Tams Elektronik (2 functions out, about 11€).

As suggested above by Thomas, you could use an FH05 from Doehler und Haass (about 18€). This decoder is a bit better: it has 4 outputs. If you install interior lighting, with a FH05 you could control it separately.

As for the locomotive with sound, I personally prefer ESU Loksound Micro v4.0 products.
If it's too big, maybe you could look at the "SL76" from Ct-Elektronik (Tran.at).
